adults with mental health needs within the UK. We have developed an excellent transitional service that aims to deliver outstanding outcomes for the individuals in our care.

Who We Support

Mental health needs
Section 117 and 17 and informal
Forensic history
Adults aged between eighteen (18) and sixty five (65)
A history of long-term hospitalisation,
Broken down placements

The individual is currently in a mental health hospital and is ready for discharge
Have complex mental disorders such as, Attachment Disorder, Atypical Depression, Obsessive Compulsive Disorder, Bipolar Disorder, Personality Disorder, Depression, Schizophrenia
The person is subject to the provisions of the CPA
Financial support needs, paying bills, shopping, budgeting, money management and accessing benefits including direct payments and personal budgets

Our Person-centred Approach

Within our supported living services we provide a person-centred approach to discharge from hospital and care planning. We ensure that we put the individual at the centre of everything we do.
Our team tailor makes the support to each individual depending on their needs.

Our Approach

to Supported Living

Supported Living is an important step towards independence. We know how important it is for individuals to have the
right support so they can live the life they want to live.

We support individuals with mental health needs and behaviours that challenge. Individuals are supported to hold their own tenancy agreement with a registered housing association. Individuals who are interested in our services, may have previously been detained within a hospital setting and are ready for a step down placement within their local community.

We have experienced, trained staff twenty four hours a day, the service benefits from a core staffing team. Each
individual would be allocated an experienced and consistent staff team and training would be bespoke to individual
support needs which enables tenants to live as independently and safely as possible. When individuals come to our
supported living services, we take the time to get to know them, their preferences, as well as their hopes and dreams for
the future. We support individuals with a personal discharge plan to achieve their goals and aspirations and ensure they
feel they are in control of their own life.

How we support individuals:

Providing a safe, comfortable and homely environment with person centred support.
Enabling individuals to build a meaningful independent life in a community setting.
Working with them to find out their strengths, abilities and preferences – We get to know them, their families, carers advocates and other people who are important to them and their care and support
Helping individuals to set goals as part of their support plan
Working with each person using the organisations assessment, care planning and review system to achieve positive outcomes for each individual.
Providing person-centred practical help for individuals, such as training and support on how to manage their home, their money, gardening, housework and healthy living
Ensuring great care has been taken to assess referrals in particular, any emergency referrals; to ensure the correct balance/matching within the existing group of service users is maintained.
Supporting service users in relation to acquiring vocational experience or academic qualifications at school/college, that would enable him/her to continue his/her education or undertake a career of his/her choice.
Assist each person, to maintain and establish positive relationships with his/her family.
